Folate Labels Demystified: Why DFE Matters
Folate labels are routinely misread. The unit "mcg DFE" is what matters and changes a 400 mcg dose into 680. Here's how to read a folate label correctly.
Folate is one of the most mislabelled nutrients in the supplement aisle. The numbers on the back of two different bottles can look the same and deliver wildly different amounts of usable folate to your body.
The reason is a unit called the Dietary Folate Equivalent, or DFE. Once you understand it, you can read a folate label correctly in about ten seconds. Until you understand it, the label is essentially nonsense.
The short version
If your bottle lists folate in straight micrograms (mcg), you don't know what dose you're actually taking.
Modern labels (post-2020 in the US, post-2021 in the UK and EU) are required to list folate in mcg DFE. The DFE number accounts for the fact that synthetic folic acid is absorbed roughly 1.7 times more efficiently than food folate.
That correction matters because most supplement folate is one of two synthetic forms (folic acid or methylfolate), and the raw mcg of those forms is not directly comparable to the mcg of folate in spinach.
Three forms, three different rules
Folate (the natural form in food)
Found in leafy greens, legumes, liver. Absorption efficiency: roughly 50%. Label units: mcg DFE = mcg of food folate.
Spinach with 100 mcg of folate contributes 100 mcg DFE to your daily intake.
Folic acid (the synthetic form in fortified foods and most supplements)
Used in fortified flour, breakfast cereals, and the vast majority of standard multivitamins. Absorption efficiency: closer to 85%, which is why a smaller raw amount produces the same blood folate effect as a larger amount of food folate.
The DFE conversion: 1 mcg of folic acid taken with food = 1.7 mcg DFE. 1 mcg of folic acid on an empty stomach = 2.0 mcg DFE.
This is why a supplement labelled "400 mcg folic acid" actually delivers 680 mcg DFE.
Methylfolate (5-MTHF or L-methylfolate)
The bioactive form, increasingly used in higher-end supplements. Doesn't require enzymatic conversion in the body, which is why it's marketed to people with MTHFR variants.
The DFE conversion: 1 mcg of methylfolate = 1.7 mcg DFE (same conversion as folic acid).
A supplement labelled "400 mcg methylfolate" delivers 680 mcg DFE.
Why this matters for the people who care about folate
Three groups have a meaningful reason to track their folate accurately.
Pregnant or trying to conceive
The recommended pregnancy intake is 600 mcg DFE per day, with 400 mcg coming from a supplement before and during early pregnancy. If your prenatal lists "800 mcg folate" without specifying DFE, you may be reading a label written under the old units, which would actually deliver around 1360 mcg DFE in modern terms. That is above the 1000 mcg upper limit set for adults.
Read the units carefully. If your bottle says "800 mcg folic acid" without DFE, that's 1360 mcg DFE, which is too high for daily long-term use.
People with MTHFR variants
Roughly 40% of the population carries at least one copy of an MTHFR variant that reduces the body's ability to convert folic acid to its active form. For these people, methylfolate is the more useful form.
But the DFE number is calculated the same way. Don't assume a methylfolate supplement is somehow more potent than a folic acid one at the same mcg DFE. The form changes how reliably your body uses it, not how much is technically delivered.
People taking methotrexate or other folate-modifying drugs
Methotrexate (used for autoimmune disease and some cancers) works partly by blocking folate metabolism. Supplemental folate (usually folic acid 5 mg weekly) is often prescribed alongside it to reduce side effects. The dose your prescriber specifies is what matters; don't add another folate supplement on top.
Reading a label correctly, in three steps
Step 1: find the folate line on the supplement facts panel.
Step 2: check the units. Is it "mcg" (old units, ambiguous) or "mcg DFE" (modern, useful)?
Step 3: if mcg DFE, the number is your actual contribution to daily intake. If mcg only, treat it as a synthetic folate dose: multiply by 1.7 to get an approximate DFE.
Some bottles list both. Look for two numbers like "680 mcg DFE (400 mcg folic acid)." That's the format you want.
Common mistakes that come from misreading the label
- Doubling up on folate from a prenatal and a separate methylated B-complex, ending up over the 1000 mcg DFE upper limit
- Buying a "natural folate" supplement at a high mcg number, not realising the DFE figure is even higher
- Assuming methylfolate at any dose is safer than folic acid (the same upper limit applies)
- Taking a 5 mg "weekly" dose daily because of label confusion
- Underdosing in pregnancy by misreading "400 mcg folate" food-folate equivalents as folic acid units
Upper limits worth knowing
The tolerable upper intake level for adults set by the EFSA and the US Institute of Medicine is 1000 mcg DFE per day from supplements and fortified foods (food folate is unlimited). Above that, two risks emerge:
- Unmetabolised folic acid in the blood, which has been associated with some negative outcomes in observational studies (the mechanism and significance are still debated)
- Masking of B12 deficiency: high folate intake can normalise the blood signs of B12 deficiency while the neurological damage continues silently
The B12 masking issue is the more established risk. If you take more than 400 mcg of supplemental folate (DFE), have your B12 status checked alongside it.
The simple rules
- Daily intake target: 400 mcg DFE for non-pregnant adults; 600 mcg DFE during pregnancy.
- Upper limit: 1000 mcg DFE per day from supplements and fortified food combined.
- Form choice: methylfolate if you know you have an MTHFR variant or your practitioner recommends it; folic acid is fine for most other people.
- Always check the units: mcg DFE is the only number you can compare across products.
