Ashwagandha: What KSM-66 and Sensoril Actually Mean

KSM-66 and Sensoril are standardized ashwagandha extracts with different plant parts, withanolide concentrations, and clinical evidence. Here is what the research shows for stress, sleep, cortisol, and strength, plus dosing, cycling, and who should avoid it.

Ashwagandha is one of the most popular adaptogen supplements on the market. It also generates more confusion than almost any other ingredient, because supplement labels don't just say "ashwagandha." They say KSM-66, Sensoril, "full-spectrum root extract," or "root and leaf extract." These are not interchangeable. They differ in plant parts used, active compound concentrations, and what the clinical research actually tested.

What ashwagandha is

Ashwagandha (Withania somnifera) is a shrub native to India, the Middle East, and parts of Africa. It has been used in Ayurvedic medicine for thousands of years, traditionally classified as a "rasayana" (rejuvenator). The plant belongs to the nightshade family, the same family as tomatoes and peppers.

The biologically active compounds in ashwagandha are called withanolides, a group of naturally occurring steroids. Withanolides are responsible for most of the effects studied in clinical trials: cortisol reduction, anxiolytic activity, and potential effects on strength and endurance.

Not all ashwagandha supplements contain the same concentration of withanolides, and this is where the confusion starts.

Root extract vs. root and leaf extract

Traditional Ayurvedic preparations used the root. The root contains withanolides, but at lower natural concentrations than the leaves. Leaves are cheaper to harvest and contain higher raw withanolide levels, which means a root-and-leaf extract can hit a higher withanolide percentage at lower production cost.

That sounds like a win, but the withanolide profile differs between roots and leaves. Root withanolides include withaferin A at low levels, along with withanolide A, withanolide D, and withanoside IV and V. Leaf extracts contain much higher levels of withaferin A, which is cytotoxic at elevated concentrations. In cell studies, withaferin A has shown anti-cancer properties, but it is also the compound most associated with gastrointestinal side effects at supplement doses.

This is why "higher withanolide percentage" does not automatically mean "better." The source matters.

KSM-66: root-only extract

What it is

KSM-66 is a branded, standardized ashwagandha extract made by Ixoreal Biomed. It uses a root-only extraction process that took 14 years to develop. The extraction method is based on "Green Chemistry" principles, using milk as part of the pre-treatment process (relevant for dairy-free consumers).

KSM-66 is standardized to a minimum of 5% withanolides by HPLC. Because it uses root only, the withanolide profile skews toward withanolide A and glycowithanolides, with minimal withaferin A.

Clinical evidence

KSM-66 has the largest body of published human clinical trials of any ashwagandha extract. Key findings:

  • Stress and cortisol: A 2012 study by Chandrasekhar et al. gave 300 mg KSM-66 twice daily (600 mg total) to 64 adults with chronic stress for 60 days. The KSM-66 group showed a 27.9% reduction in serum cortisol compared to placebo. Perceived stress scores (PSS) dropped by 44%.
  • Sleep: A 2019 study by Langade et al. found that 600 mg/day of KSM-66 for 10 weeks improved sleep quality scores (measured by Pittsburgh Sleep Quality Index) significantly compared to placebo. Sleep onset latency decreased and total sleep time increased.
  • Strength and recovery: Wankhede et al. (2015) studied 57 young males doing resistance training. The group taking 300 mg KSM-66 twice daily for 8 weeks saw significantly greater increases in muscle strength (bench press and leg extension) and muscle size, along with greater reductions in exercise-induced muscle damage and body fat percentage compared to placebo.
  • Testosterone: The Wankhede study also noted a significantly greater increase in testosterone in the KSM-66 group. A separate study (Lopresti et al., 2019) in overweight men aged 40-70 found 600 mg/day KSM-66 for 8 weeks increased DHEA-S by 18% and testosterone by 14.7% compared to placebo.

Typical dosing

Most KSM-66 studies use 300-600 mg per day, usually split into two doses. The 600 mg/day dose has the most evidence behind it.

Sensoril: root and leaf extract

What it is

Sensoril is a branded extract made by Natreon, Inc. Unlike KSM-66, Sensoril uses both root and leaf material. It is standardized to a minimum of 10% withanolides and 32% oligosaccharides by HPLC.

The higher withanolide percentage reflects the leaf contribution. Sensoril's withanolide profile includes more withaferin A than KSM-66, though Natreon's extraction process is designed to keep this within safe ranges. The oligosaccharide standardization is unique to Sensoril and may contribute to its calming effects.

Clinical evidence

Sensoril has fewer published trials than KSM-66 but still has meaningful human data:

  • Stress and anxiety: Auddy et al. (2008) compared three doses of Sensoril (125 mg, 250 mg, and 500 mg daily) against placebo in 130 adults with moderate-to-severe anxiety. All three doses reduced anxiety scores significantly. The 250 mg dose showed the best cortisol reduction, lowering it by 24.2% versus placebo.
  • Cardiovascular markers: The same study found that Sensoril at 250 mg/day reduced C-reactive protein (a marker of inflammation) by 36% and lowered fasting blood glucose levels.
  • Cognitive function: Pingali et al. (2014) found that 250 mg Sensoril twice daily improved reaction time, cognitive task performance, and attention in healthy adults over 14 days.

Typical dosing

Clinical studies use 125-250 mg per day, significantly lower than KSM-66 doses. This is because the higher withanolide concentration means less material is needed for a similar effect. The 250 mg/day dose is most commonly studied.

KSM-66 vs. Sensoril: which to choose

Neither is objectively better. They are optimised for different profiles:

  • KSM-66 has more published trials overall, particularly for strength, testosterone, and sports performance. If your primary goal is exercise performance or recovery, KSM-66 at 600 mg/day has the stronger evidence base.
  • Sensoril tends to produce a more calming, sedating effect, likely due to the oligosaccharide content and different withanolide profile. If your primary goal is stress reduction, anxiety, or sleep, Sensoril at 125-250 mg/day is worth considering.
  • Both have solid evidence for cortisol reduction. The difference is roughly 28% (KSM-66) vs. 24% (Sensoril) in their respective trials, which is within noise given different study populations.

One practical note: because Sensoril uses lower doses, products containing it tend to leave more capsule space for other ingredients. In combination formulas, Sensoril is easier to include at a clinically studied dose.

Generic ashwagandha extracts

Many supplements use unbranded ashwagandha extract, sometimes labelled as "root extract" or "root powder." These products vary widely in withanolide content and are not backed by the clinical trials that used KSM-66 or Sensoril specifically. A product claiming "5% withanolides" from an unbranded extract may or may not deliver consistent potency across batches.

If cost is a concern, unbranded root extracts can still be effective. But you lose the manufacturing consistency and third-party verification that the branded extracts provide. Look for products that list their withanolide percentage and have a certificate of analysis (CoA) available.

Cycling ashwagandha

Ashwagandha is generally well-tolerated in studies lasting 8 to 12 weeks. Most clinical trials run for this duration. There is less long-term safety data beyond 12 weeks of continuous use.

The common recommendation is 8-12 weeks on, followed by 2-4 weeks off. This cycling pattern comes partly from traditional Ayurvedic practice and partly from the precautionary principle given the limited long-term data.

Some practitioners suggest cycling is especially relevant because ashwagandha affects thyroid hormone levels. Studies have shown increases in T3 and T4 levels, which is beneficial for hypothyroid individuals but potentially problematic for those with normal or elevated thyroid function over extended periods.

Who should avoid ashwagandha

Ashwagandha is not appropriate for everyone:

  • Pregnant or nursing: Ashwagandha has been associated with abortifacient effects in animal studies. There is no human safety data for pregnancy. Avoid it.
  • Autoimmune thyroid conditions: Ashwagandha can stimulate thyroid hormone production. For people with Hashimoto's thyroiditis or Graves' disease, this can interfere with medication dosing and disease management. Consult your endocrinologist before use.
  • Nightshade sensitivity: Ashwagandha is a member of the Solanaceae (nightshade) family. If you react to tomatoes, peppers, or eggplant, you may also react to ashwagandha.
  • Immunosuppressant medications: Ashwagandha has immunomodulatory effects and may interfere with drugs that suppress the immune system, including medications used after organ transplants.
  • Surgery: Ashwagandha may affect anaesthesia and blood sugar regulation. Most guidance recommends stopping it at least two weeks before scheduled surgery.

Practical takeaways

  • Check the label for KSM-66 or Sensoril by name. Generic "ashwagandha extract" can be fine, but the branded forms give you manufacturing consistency and direct alignment with clinical trials.
  • For exercise performance and testosterone: KSM-66 at 600 mg/day has the most evidence.
  • For stress, anxiety, and sleep: either works, but Sensoril at 250 mg/day may have a slight edge for calming effects.
  • Cycle your use: 8-12 weeks on, 2-4 weeks off.
  • If you take thyroid medication, talk to your doctor first.
  • Ignore products using "proprietary blend" labels that hide the actual ashwagandha dose. You need to know the dose to compare against what was tested in studies.
