Biotin for Hair and Nails: Separating Marketing from Evidence
What biotin actually does, who is truly deficient, what the evidence says about hair and nail supplementation, the lab test interference problem, and reasonable dosing.
Biotin is one of the most marketed supplements in the beauty and wellness space. Walk into any pharmacy and you will find biotin supplements promising thicker hair, stronger nails, and healthier skin. The doses range from 1,000 to 10,000 mcg, often labelled as 'high-strength' or 'maximum potency.' The adequate intake for biotin is 30 mcg per day.
That is a gap of 30 to 300 times the recommended amount. Is there evidence that these mega-doses do anything for people who are not deficient? The honest answer is: not much.
What biotin actually does in the body
Biotin (vitamin B7) is a cofactor for five carboxylase enzymes. These enzymes are involved in fatty acid synthesis, amino acid metabolism, and gluconeogenesis. Without adequate biotin, these metabolic pathways do not function properly.
Keratin, the structural protein in hair and nails, depends on normal biotin-dependent enzyme activity for its production. This is the biological basis for the connection between biotin and hair and nail health. It is real, but the conclusion most marketers draw from it is not.
The logic goes: biotin is needed for keratin production, therefore more biotin means more or better keratin. This does not follow. Biotin-dependent enzymes, like most enzymes, have a saturation point. Once the enzyme has enough biotin to function at full capacity, additional biotin does not increase its activity. It is like filling a fuel tank: once it is full, pouring in more petrol does not make the car go faster.
Who is actually deficient in biotin?
True biotin deficiency is rare in developed countries. Biotin is found in a wide range of foods (eggs, nuts, seeds, salmon, sweet potatoes, spinach) and is also produced by gut bacteria. The adequate intake of 30 mcg per day is easily met by a normal diet.
Groups at genuine risk of deficiency include:
- People taking certain anticonvulsants: carbamazepine, phenytoin, and phenobarbital accelerate biotin metabolism. Long-term use can lead to subclinical deficiency.
- Chronic alcohol use: alcohol impairs biotin absorption and increases urinary excretion.
- Pregnancy and breastfeeding: biotin requirements increase, and marginal deficiency has been documented in up to a third of pregnant women. This is usually subclinical.
- People who consume large amounts of raw egg whites: avidin, a protein in raw egg whites, binds biotin and prevents absorption. Cooking denatures avidin, so this only applies to raw consumption. You would need to eat several raw eggs daily for this to matter.
- Biotinidase deficiency: a rare genetic condition (roughly 1 in 60,000) that impairs biotin recycling. It is screened for at birth in most countries.
For these groups, biotin supplementation at modest doses (100 to 300 mcg) is reasonable and evidence-supported. The issue is that these groups represent a small fraction of the people buying biotin supplements.
What the evidence says about hair growth
The evidence for biotin supplementation improving hair growth in non-deficient people is weak. This is not a matter of interpreting data optimistically or pessimistically. The high-quality evidence simply does not exist.
A 2017 systematic review in Skin Appendage Disorders examined all published studies on biotin and hair or nail changes. The authors found 18 reported cases where biotin supplementation improved hair or nail conditions. In every case, the patient had an underlying condition associated with biotin deficiency or poor biotin metabolism. There were no controlled trials demonstrating benefit in people with normal biotin status.
Several industry-funded studies have tested proprietary blends containing biotin alongside other ingredients (marine collagen, zinc, iron, saw palmetto, horsetail extract). Some of these showed improvements in hair thickness or growth, but it is impossible to attribute the result to biotin specifically when it is one ingredient in a multi-component blend.
The best summary of the current evidence: if you are biotin-deficient, supplementation will help your hair and nails. If you are not deficient, there is no reliable evidence that it will.
The nail evidence is slightly better
Biotin's strongest evidence, while still limited, is for brittle nails. A 1993 study in the Journal of the American Academy of Dermatology gave 2,500 mcg of biotin daily to 45 patients with brittle nails. After an average of 5.5 months, 63% showed clinical improvement in nail thickness.
This study had no placebo control, which is a significant limitation. Nail conditions fluctuate, and some improvement would be expected over 5.5 months regardless of treatment. But the result was consistent with earlier veterinary research (biotin has been used for years to strengthen horse hooves), and the biological plausibility is strong.
If you have genuinely brittle, splitting nails and want to try biotin, a dose of 2,500 mcg daily for 3 to 6 months is the protocol used in the available research. Expect to wait at least 3 months for visible changes because nail growth is slow: fingernails grow about 3.5 mm per month.
The lab test interference problem
This is the most underreported issue with high-dose biotin supplements. Biotin interferes with a common type of laboratory assay called immunoassay, which is used to measure thyroid hormones, troponin (a cardiac marker), parathyroid hormone, cortisol, and several other important lab values.
High-dose biotin (5,000 mcg and above) can cause falsely elevated or falsely depressed results depending on the specific test. The consequences are not theoretical:
- Thyroid tests: biotin can cause falsely low TSH and falsely high free T4 and T3, mimicking the pattern of Graves' disease (hyperthyroidism). People have received unnecessary thyroid treatment based on lab results skewed by biotin supplements.
- Troponin: biotin can cause falsely low troponin results, which could mask a heart attack in an emergency setting. The FDA issued a safety communication about this in 2017.
- Hormone panels: cortisol, testosterone, estradiol, and other hormone levels can be affected, leading to misdiagnosis.
The FDA recommends stopping biotin supplements at least 72 hours before blood tests. Many healthcare providers do not ask about biotin use, and many patients do not mention it because they consider it a harmless vitamin, not a medication. If you take high-dose biotin and are scheduled for blood work, tell your doctor and stop the supplement at least 3 days beforehand.
Reasonable dosing if you choose to try it
If you decide to try biotin despite the limited evidence, here is a pragmatic approach:
- For general maintenance: 30 mcg per day (the adequate intake). This is easily obtained from diet. A supplement is unnecessary for most people.
- For brittle nails: 2,500 mcg per day for 3 to 6 months, based on the available research. Reassess after 6 months. If no improvement, it is likely not going to help.
- For hair thinning: rule out other causes first. Iron deficiency, thyroid dysfunction, vitamin D deficiency, and hormonal changes are all more common and more treatable causes of hair loss. If all are normal and you still want to try biotin, 2,500 to 5,000 mcg is the range used in most studies.
Doses above 5,000 mcg have no additional evidence supporting them and increase the risk of lab test interference. The 10,000 mcg supplements sold in shops are not supported by dose-response data.
Better-supported alternatives for hair and nails
If you are concerned about hair or nail health, several factors have stronger evidence than biotin for non-deficient people:
- Iron: iron deficiency (even without full anaemia) is a well-documented cause of hair loss, particularly in women. Ferritin levels below 30 ng/mL are associated with increased shedding. Correcting iron deficiency often improves hair growth within 3 to 6 months.
- Vitamin D: vitamin D receptors are present in hair follicles, and deficiency is associated with alopecia areata and telogen effluvium. Given that vitamin D deficiency is common (particularly in the UK), checking and correcting levels is a higher-yield intervention than biotin.
- Zinc: zinc deficiency causes hair loss, and supplementation in deficient individuals restores growth. The evidence is specific to deficiency, not to general supplementation.
- Protein intake: hair is 95% keratin, which is a protein. Inadequate protein intake, common in restrictive diets, contributes to thinning. Ensuring 0.8 to 1 g of protein per kg of body weight daily supports normal hair growth.
- Addressing the underlying cause: thyroid disorders, hormonal changes (postpartum, menopause, PCOS), autoimmune conditions, and stress are all more common causes of hair loss than any nutrient deficiency. A GP or dermatologist can run the relevant tests.
The bottom line
Biotin is essential for health, and deficiency genuinely does affect hair and nails. But deficiency is rare. The mega-dose supplements lining pharmacy shelves are backed by marketing budgets, not clinical trials. The strongest evidence for supplementation in non-deficient people is for brittle nails, and even that evidence has significant limitations.
