ADHD and Supplements: What the Evidence Actually Says
Reddit stacks 15 things. The evidence backs 5. A dose-by-dose read on what actually works for ADHD supplementation, what to skip, and how to layer alongside stimulant medication safely.
Search ADHD and supplements on Reddit and you get a shopping list of 15 things. Omega-3, magnesium threonate, L-tyrosine, saffron, rhodiola, bacopa, citicoline, phosphatidylserine, lion's mane, ginkgo, ashwagandha, NAC, zinc, iron, methylated B-complex. Every thread ranks them differently, half the doses cited are wrong, and the vibe is that if you just stack enough of them you can build your own Adderall. You cannot.
The evidence-backed core for ADHD is small. Five or six ingredients have real human trials in ADHD populations, not just theoretical mechanism papers. The rest are nootropics that might help cognition in general, which is a different claim. This post separates the two, cites the doses that were actually studied, and covers the safety points that matter if you take a prescribed stimulant.
One thing to say up front: supplements are not a substitute for evaluation, therapy, or medication if you need them. What they can do is fix the deficiencies that make ADHD worse and smooth some of the edges around a stimulant. That is a smaller, more honest goal than what most stack videos promise.
Why ADHD brains respond to some supplements at all
The neurochemistry ADHD medication targets is dopamine and norepinephrine signalling in the prefrontal cortex. Methylphenidate and amphetamines increase the availability of both. Supplements do not touch those systems with anything close to the same force. But several nutrients act as cofactors for the enzymes that make dopamine and norepinephrine, and several others modulate the systems that support attention: cell membrane integrity, GABA tone, and iron-dependent tyrosine hydroxylase activity.
Two nutrients matter enough that adult ADHD populations show measurable deficiencies compared to controls: iron (specifically ferritin) and omega-3 fatty acids. Correcting either does not treat ADHD but does move the needle on the symptoms in a subset of people. That is the honest ceiling on what supplementation can do.
The evidence-backed core
Five supplements have direct trials in ADHD populations with enough replication to be worth taking seriously. In rough order of evidence strength:
1. Omega-3, EPA-heavy
Meta-analyses of omega-3 supplementation in ADHD (Bloch and Qawasmi 2011, Chang et al. 2018) show a small but consistent reduction in inattention and hyperactivity scores. Effect sizes are around 0.3, roughly a third of what stimulant medication produces. The active fraction is EPA, not DHA. Trials using at least 500mg EPA daily consistently outperform DHA-heavy formulations.
- Dose: 1000 to 2000mg EPA per day. Read the label, not the total omega-3 number. A 1000mg fish oil capsule often contains 180mg EPA and 120mg DHA.
- Form: Triglyceride or re-esterified triglyceride form absorbs meaningfully better than ethyl ester. Cod liver oil is not concentrated enough at reasonable volumes.
- Timeline: Effects show up at 8 to 12 weeks, not the first week. Trials that ran under 8 weeks tended to show no effect.
2. Iron, with ferritin as the actual target
Ferritin below 30 ng/mL is common in ADHD populations, including in people whose haemoglobin is normal. Iron is a cofactor for tyrosine hydroxylase, the rate-limiting enzyme in dopamine synthesis. Low ferritin correlates with worse ADHD symptoms and worse response to stimulant medication in several trials (Konofal et al. 2004, Cortese et al. 2012).
- Test first: Do not supplement iron blindly. Get a serum ferritin test. The lab reference range starts at 15 ng/mL but for cognitive symptoms the target is 50 to 100 ng/mL.
- Dose if low: 30 to 65mg elemental iron daily. Bisglycinate is easier on the gut than sulphate. Take on an empty stomach with vitamin C for absorption, away from coffee, tea, calcium, and zinc.
- Retest at 12 weeks: Ferritin moves slowly. Do not keep dosing indefinitely without a follow-up test, iron overload has its own problems.
3. Zinc
Zinc is another cofactor in dopamine metabolism, and low zinc correlates with worse ADHD symptoms. Two RCTs (Bilici et al. 2004, Akhondzadeh et al. 2004) showed zinc supplementation modestly improved ADHD ratings, though the effect is smaller than omega-3 or iron correction. The strongest signal is in populations with baseline low zinc, which tracks with regions where deficiency is common.
- Dose: 15 to 30mg zinc picolinate or bisglycinate. Higher doses risk copper deficiency over months; pair with 1 to 2mg copper if you supplement zinc long-term.
- Timing: Take with food to reduce nausea. Keep away from iron and calcium by at least 2 hours.
4. Magnesium
Magnesium's role in ADHD is less about direct dopamine modulation and more about the physiological load stimulants put on the system: sleep quality, muscle tension, jaw clenching, and cortisol. Trials specifically in ADHD are smaller and less clean than the omega-3 or iron literature, but the safety profile and the sleep and anxiety improvements make it a defensible add.
- Dose: 200 to 400mg elemental magnesium. Glycinate for general use and sleep. Threonate has better central nervous system penetration in animal studies but the human trials are thin and it is expensive per mg.
- Timing: Evening dose for sleep. Split doses if you go over 300mg to reduce gut effects.
5. Vitamin D
Not ADHD-specific, but low vitamin D is associated with worse mood, worse cognitive performance, and worse response to stimulants. Correction if deficient is one of the cheapest wins available.
- Test first: Serum 25-hydroxyvitamin D. Target 30 to 50 ng/mL (75 to 125 nmol/L).
- Dose if low: 2000 to 5000 IU daily with K2 MK-7 100mcg. Take with a meal containing fat.
Nootropic add-ons with real trials
These sit outside the core because they are not correcting deficiencies. They are pharmacological interventions with plant compounds or amino acids that have RCT evidence in ADHD or attention outcomes. Effects are real but smaller and more variable across people.
Saffron, 28 to 30mg standardised extract
The most surprising entry. Two Iranian RCTs (Baziar et al. 2019, Khaksarian et al. 2021) compared saffron to methylphenidate in children with ADHD and found saffron non-inferior at 6 weeks. Adult data is thinner but the mechanism, likely serotonin and dopamine modulation via safranal and crocin, is plausible. Standardised extracts (affron, satiereal) at 28 to 30mg daily are the studied doses. This is the closest any supplement gets to a stimulant-like effect and it is worth trying if stimulant medication is not an option.
L-tyrosine, 500 to 2000mg
Dopamine precursor. The trial evidence for ADHD specifically is weak. The stronger evidence is for cognitive performance under stress and sleep deprivation (Deijen et al. 1999, Neri et al. 1995). Popular in the ADHD community as a stimulant alternative on non-medication days or to smooth the come-down. Take on an empty stomach, at least 30 minutes away from protein. Do not stack with a stimulant medication without discussing it with your prescriber.
L-theanine, 200mg
Does not treat ADHD. Does reliably reduce the anxiety and jitteriness that stimulants and caffeine cause. If your stimulant makes you feel wired more than focused, L-theanine at 200mg alongside your morning dose smooths the edge without blunting the therapeutic effect. Evidence is strong for combined caffeine and L-theanine attention studies; the extrapolation to prescription stimulants is off-label but low risk.
Citicoline (CDP-choline), 250 to 500mg
Precursor to acetylcholine and phosphatidylcholine. One small RCT (McGlade et al. 2012) showed attention improvements in adolescents at 250 to 500mg over 28 days. Better evidence for age-related cognitive decline than ADHD specifically. Alpha-GPC is a related choline source with similar mechanism, some prefer it for a subjectively cleaner effect.
What the community piles on that the evidence does not support
Nootropic communities stack a lot of compounds that have interesting animal data or mechanism papers but almost no human ADHD evidence. This is not to say they never work, but if your stack is 12 things long, most of them are here.
- Racetams (piracetam, aniracetam, phenylpiracetam): No RCT evidence for ADHD. Phenylpiracetam has stimulant-like effects that some people find useful, but tolerance builds fast and it is a banned substance in most tested sports.
- Lion's mane: Nerve growth factor evidence in mice. Human cognitive trials are on mild cognitive impairment in older adults, not ADHD. Not harmful, but not doing what people think it is doing.
- Bacopa monnieri: Some memory evidence in older adults at 12 weeks. In ADHD specifically the data is very thin. The saponins can cause gut effects in a subset of people.
- Ashwagandha: Anxiety and cortisol data at 300 to 600mg KSM-66. Useful for stress, not for attention. Do not stack with a stimulant without noting the opposing pharmacology.
- Nootropic mega-stacks: Stacks of 8 or more compounds simultaneously make it impossible to tell what is working. Start with one intervention at a time.
- Mega-doses of anything: More is not better. The RCT doses above are the doses that produced results. Doubling them does not double the effect and often introduces side effects.
Safety points if you take a prescribed stimulant
Adderall, Vyvanse, Concerta, Elvanse and Ritalin are the medications this section is talking about. The core interactions worth flagging:
- L-tyrosine plus a stimulant: Not automatically dangerous but the pharmacology stacks in the same direction. Higher perceived stimulation, higher heart rate, potentially higher blood pressure. Do not start a high L-tyrosine dose on a day you have a stimulant on board without talking to your prescriber first.
- Iron and coffee or your stimulant: Coffee and calcium reduce non-heme iron absorption. If you take iron in the morning next to your stimulant and coffee, you are wasting most of the iron. Split them by at least 2 hours.
- Magnesium and cramping or bruxism: Jaw clenching and muscle tension are common stimulant side effects. Evening magnesium glycinate at 200 to 400mg helps both, and separately supports sleep, which stimulants can shorten.
- Rhodiola, ashwagandha, and adaptogens: Cortisol modulators can interact with the sympathetic activation of stimulants. Effects are usually mild but track how you feel when adding one, do not add two adaptogens the same week.
- St John's wort: Do not take with any stimulant or SSRI. Serotonin syndrome risk plus significant CYP450 induction that will change how your other medications are metabolised.
How to actually test if a stack is working
The single most common mistake in ADHD supplement stacks is adding four things the same week and having no idea which one, if any, is helping. The disciplined version is boring and works:
- Baseline first: Ferritin, vitamin D, and if you have insurance for it a full micronutrient panel. Correct the deficiencies before layering nootropics on top.
- One change at a time: Add a single supplement, hold it for 4 to 8 weeks depending on the mechanism (nutrients need longer, nootropics can show up in 2), then decide.
- Pre-decide the metric: Written before you start. Adult ADHD Self-Report Scale (ASRS-v1.1), tasks completed per day, subjective focus 1 to 10 logged three times a day, sleep score off your wearable. Pick two and stick with them.
- Retest bloodwork: Ferritin and vitamin D at 12 weeks. Confirm the intervention is doing what you thought.
- Drop what is not doing anything: Half of what you start does not survive a real 4-week trial. That is the point of running one.
Where community stacks converge
The stacks discussed in r/ADHD, r/Nootropics, the Huberman Lab ADHD episode, and Peter Attia's newsletter converge on a smaller set than the internet suggests. The overlap across all four is EPA-heavy omega-3, magnesium glycinate, and ferritin correction if bloodwork calls for it. That is the practical core almost every serious source agrees on. Everything else, including saffron, L-tyrosine, and citicoline, sits in the personal-experiment tier.
If you are starting from zero, the honest recommendation is: test ferritin and vitamin D, correct if low, add 1000 to 2000mg EPA and 200 to 400mg magnesium glycinate, run for 12 weeks, retest. That is not a viral stack but it is the one with the evidence behind it.
